AFM CONTRACT FORMS
The four AFM Contract Forms contained on this page are available for download to members of The Greater Lancaster Federation of Musicians, Local 294 AFM. The three "L" forms are for local engagements only; meaning engagements held in the geographical jurisdiction of Local 294 which is the entirety of Lancaster County PA. The "L" Contract Forms are filed with the Secretary-Treasurer of Local 294. The "T" form is for engagements held outside the jurisdiction of Local 294 ("travelling").
The Form L-1 may be used for single or multiple dates. No provision for pension contributions to the AFM&EPF (American Federation of Musicians and Employers' Pension Fund) is provided on this form. If a collective bargaining or participation agreement exists between Local 294 and the employer of the musicians that provides for pension contributions, the Form LP-1 must be utilized. This form may be obtained from your President or Secretary-Treasurer.

The Form L-2 is exactly the same as the L-1 with one exception. The L-1 provides an option for the Purchaser to select an arbitration procedure in the event of any controversy or claim.
The Purchaser must select either the American Arbitration Association or the Executive Board of the Local Union to settle any disputes. The L-2 does not provide this option to the Purchaser.

The Form LS-1 may be used for a single engagement* only. This contract form is for the purpose of facilitating pension contributions on behalf of individual musicians to the AFM&EPF when no collective bargaining or participation agreement exists.
*A single engagement is defined in Question 2 in the Question and Answers segment below.

The Forms T-2 and TP-2 must be utilized by Local 294 members and groups who reside in Lancaster County but whose engagements are outside the jurisdiction. These "travelling" contract forms must be filed with the Secretary-Treasurer of the Local in whose jurisdiction the engagement covered is to be performed. The Form T-2 may be downloaded by clicking below. The Form TP-2 may be obtained from your President or Secretary-Treasurer.
